Hope House is a 12-bed men’s board and care home for conserved clients who are able to step down to a lower level of care but who still benefit from additional support. Sonoma County’s Coordinated Entry system employs a Housing First model that prioritizes individuals, families and transition-aged-youth facing the highest vulnerability and needs for permanent, supportive housing.
