Hospital Liaison Services

Monitor psychiatric hospitalizations for all Medi-Cal beneficiaries in the County
Work with outpatient teams and community providers to ensure continuity of care
Collaborate with hospital social workers for a safe discharge
Arrange intake or follow-up appointments post-discharge
Facilitate step-down referrals to the crisis residential units
Arrange transportation from hospitals to home or placement
