Mental Health Services for Adults

Mental Health Hotlines

Our mental health programs serve seriously ill adults age 18 and older throughout Sonoma County. Services are provided by County staff, community health centers, and a network of contracted staff:

Community-based Services

Our Access Team is the first contact to request mental health services, and can be reached at (707) 565-6900.

We provide services to adults with serious and persistent mental illness and co-occurring substance use disorders, who currently do not receive comprehensive services.

We provide these services to people with severe and persistent mental illness:

  • Assessment
  • Case planning and management
  • Crisis intervention
  • Medication support
  • Therapy
  • Rehabilitation, including supportive housing and employment
  • Referral
  • Linkage

Our multidisciplinary teams provide the services:

Our Full Service Partnerships (FSPs) are multidisciplinary teams that provide intensive field-based specialty mental health services targeted at specific populations with a service commitment of doing “whatever it takes":

Assertive Community Treatment is contracted through Telecare Sonoma ACT and provides intensive specialty mental health services similar to a full service partnership, targeting mental health clients with serious mental illness who are at risk for placement at an extended care facility.

  • SonomaWORKS provides mental health and substance use disorder services to adults referred from Sonoma County Human Services Department as part of the Welfare-to-Work program.

Forensic Services

Clients are referred to Forensic Services by the court system.

Forensic Services offers specialty services to help people who present a risk to the public as determined by the Court:

  • Forensic Assertive Community Treatment Team (FACT) - full-service partnership that works with a probation officer to provide community-based specialty mental health services to people referred through Mental Health Court.
  • AB 109 Mental Health/Probation Program - services for adult offenders.
  • 1370 Outpatient Restoration Program - restoration services to individuals deemed incompetent to stand trial.

Transitional Recovery Services

Our Transitional Recovery team provides ongoing supervision and support to adults who require extended facility-based treatment, offering an opportunity to stabilize and prepare for community discharge.

We provide the highest level of care to the county’s at-risk behavioral health clients recovering in both locked and out-of-county treatment facilities, working diligently to support their recovery in the least restrictive environment.

  • Housing - residential treatment and housing options to clients receiving specialty mental health services. These options are offered in collaboration with community providers.
  • Employment: -  employment options for mental health clients who are receiving specialty mental health services. These options are offered in collaboration with community providers.
