2025-2026 Benefits Annual Enrollment
Annual Enrollment is a time to review your family's changing needs.
Take advantage of the annual enrollment period to evaluate your existing coverage and decide whether to continue with your current choices or make a change.
Elections made during the 2025-2026 Benefits Annual Enrollment period are effective June 1, 2025 - May 31, 2026.
Annual Enrollment Dates:
All Employees, Extra Help and Retirees - March 24 - April 11, 2025
Employee Self Service (ESS):
Employees and Extra Help must use Employee Self Service to make election changes March 24 - April 11, 2025.Need assistance with Employee Self Service? The instructions can help.
Retirees making a change must complete a Retiree Enrollment/Change form and submit it to the HR Benefits Unit between March 24th and April 11th. Forms cannot be turned in early.

Enrolled in Dependent Life?
Review the guidelines for dependent eligibility on page 8 of the Employee Benefits Guide. If you no longer have eligible dependents, Annual Enollment is the time to decline this coverage. Not sure if you are enrolled? You can verify your benefit coverages in Employee Self Service (ESS).
Be sure to update your beneficiaries!
If your beneficiary has changed or it has been more than 3 years since you last updated your beneficiary, now is the time to complete a new form! You can find the appropriate form on our website at https://sonomacounty.ca.gov/benefit-forms-plan-documents-and-important-information. Your beneficiaries can be updated at anytime throughout the year.
What can I do during Annual Enrollment?
- Enroll, waive coverage (due to enrollment in other group coverage or Covered CA), or decline coverage
- Change your medical plan and/or dental plan coverage level
- Enroll or change your supplemental life insurance election
- Add, waive (due to enrollment in other group coverage or Covered CA), or drop medical, dental, vision and/or dependent life insurance coverage for your dependents
How Do I Make Changes?
- Login to Employee Self Service (ESS)
- If you do not have access to a computer, contact your Payroll Clerk or the HR Benefits Unit at benefits@sonoma-county.org for an Enrollment Form
What If I Want To Keep My Current Election?
If you take no action your current benefit elections will continue effective June 1, 2025 through May 31, 2026. Be aware that making no change is considered an election to retain the benefits currently in place for the upcoming plan year. At minimum, we recommend you verify the plans you are enrolled in and the contributions for the new plan year, verify and confirm your dependents on each plan, and ensure that only your eligible dependents are cov

What Can I Do During Annual Enrollment?
- Enroll in medical coverage
- Change medical plans
- Add/drop/waive dependents from your medical coverage
- Decline medical coverage
How Do I Know If I Am Eligible?
- Currently enrolled in County offered medical benefits due to prior eligibility.
- If you have met initial qualifications in the past, and have remained in continuous active employment since, you are eligible to enroll in medical benefits during annual enrollment. Initial qualification is met when you have worked 80 hours or more in two consecutive pay periods and are regularly scheduled to work 40 hours or more per pay period.
Your enrollment in medical benefits is subject to verification of eligibility per your applicable MOU or Salary Resolution
What Am I Eligible For?
- Medical - limited to Kaiser Permanente, Sutter Health Plus and Western Health Advantage
- Employee Assistance Program (EAP)
- CareCounsel Health Advocacy Program
You do not need to elect EAP or CareCounsel. EAP is provided to all Extra Help employees at the start of employment. You will automatically be enrolled in the CareCounsel Health Advocacy Program once you become eligible.
Things To Know
Per your MOU or the Salary Resolution, the County contribution to your elected medical plan is determined by the number of hours you work in that specific pay period. You will receive the full County contribution if you work 40 or more hours in a pay period. For each pay period in which an Extra Help employee works more than 20 hours, but fewer than 40 hours, the County contribution will be pro-rated, which means that the amount of the County contribution each pay period will vary as you increase or decrease your hours. If you work less than 20 hours in a pay period, no County contribution will be made.

What Can I Do During Annual Enrollment?

Note: You may cancel or decline your medical coverage for yourself and/or dependents at any time throughout the year. Once canceled or declined you are no longer eligible for County sponsored medical benefits now or in the future.
How Do I Make A Change?
Complete a Retiree Enrollment/Change form and return it to the HR Benefits Unit, between March 24th and April 11th. Forms cannot be received prior to the start of the Annual Enrollment period beginning March 24, 2025.
