The employee/applicant is responsible for notifying the supervisor or interviewer that he/she has a disability or medical condition which requires reasonable accommodation. Notice of a disability may come in the form of:
- The employee/applicant’s direct statement to a supervisor/interviewer that he/she is unable to perform a duty that is part of the job because of a disability;
- The employee/applicant’s direct request for an accommodation to the supervisor/interviewer or
- The supervisor’s/interviewer’s receipt of information regarding an employee/applicant’s disability or need for accommodation.
