HCBA is for clients of all ages who are medically fragile, with complex physical conditions that require more skilled care.
As an alternative to nursing home or institutional living, the program helps put supportive services in place to allow them to stay at home. HCBA can also support residents wishing to return home from a nursing home or institutional facility.
A social worker and a public health nurse team determine the services clients need. Client and family/caregiver are vital partners in planning and carrying out the care management plan.
The team regularly visits clients at home to provide ongoing support, information and referrals to services and help with government and other agency programs and benefits. The team also helps to arrange, coordinate and monitor recommended medical and community services, including nursing care and some personal care services.
Please note: The number of clients HCBA can support is limited. Currently, there is a waiting list to receive services.
