  • Ring of Hands

    Accident/Incident investigations are important tools used to identify safety concerns, determine how an incident could have been prevented and to make recommendations to prevent recurrences.  Departments must follow the Risk Management accident/incident investigation procedures.

  • Incident Report with Pen

    Medical Incident Reporting - All injuries/illnesses/exposures must be reported to the Disability Management Unit within 24 hours.  Supervisors and Managers are responsible to complete the Supervisor’s Report of Occupational Injury/Illness/Exposure form and forward it to Disability Management with a copy to their corresponding Department Safety Coordinator and others, according to the Department’s process.

    When reporting non-injury accidents/incidents, complete the entire accident/incident report within 24 hours of the accident/incident. If you are injured and unable to complete the report, your department should report it for you. After it is completed, please bring it to Fleet Operations for the formal report.
