  • These community agencies are under contact with DHS to provide addiction treatment services
  • The Crisis Assessment, Prevention, and Education Team (CAPE Team) is a prevention and early intervention strategy specifically designed to intervene with transition age youth ages, 16 to 25, who are at risk of or are experiencing first onset of serious psychiatric illness and its multiple issues and risk factors
  • Sonoma County Eligible Professionals (EP) and Eligible Hospitals (EH) and Critical Access Hospitals (CAH) may meet their Public Health Reporting, meaningful use objectives.

  • Our mental health programs serve seriously ill adults age 18 and older throughout Sonoma County. Services are provided by County staff, community health centers, and a network of contracted personnel.
  • Seriously emotionally disturbed (SED) children and adolescents to age 18 are served by a system of care that includes County staff working in partnership with Schools, the Valley of the Moon children's home, juvenile health services at Los Guilicos, and community partners.
