Linkages / Multipurpose Senior Services Program

These two programs offer case management and other services that help older adults remain safely and independently at home, rather than moving into a nursing facility or residential care.

Linkages social workers help guide clients to services and support in the community.

The MSSP Care Management Team includes a social worker and a public health nurse, who coordinate, arrange and monitor services to meet medical care and social needs.

Who is Eligible for Linkages?

Sonoma County residents who are:

  • Age 60 or older
  • Any income level. Some clients pay a share of cost
  • Need assistance to live independently due to illness, injury or disability
  • Need help managing their care and getting community services
  • Not receiving case management from the Department of Rehabilitation, Department of Mental Health or Department of Developmental Disabilities

Services Through Linkages

Promotes clients’ independence and well-being by empowering them to put in place needed new resources, such as:

  • An assessment of needs for support and a plan for how to meet those future needs as your condition or situation changes
  • Referrals to local non-profit and government programs and services, such as transportation, caregiver support and home-delivered meals
  • Supportive in-home and telephone contacts and a support network

Who is Eligible for MSSP?

  • Sonoma County residents
  • 65 years of age or older
  • currently eligible for Medi-Cal (without any Medi-Cal share of cost)
  • functionally impaired and at risk of institutionalization in a nursing facility
  • can be served at home within MSSP budget limitations
  • willing to accept MSSP services

  • In-Home Supportive Services
  • Accompaniment to medical and other appointments
  • Medical Equipment
  • Home-Delivered Meals
  • Respite Care for Caregivers
  • Specialized Transportation
  • Counseling
  • Communication Devices
  • Minor Home Safety Repairs
  • Money Management Support
  • Some Supplies and Non-Medical Equipment
