Linkages / Multipurpose Senior Services Program

These two programs offer case management and other services that help older adults remain safely and independently at home, rather than moving into a nursing facility or residential care.

Linkages social workers help guide clients to services and support in the community.

The MSSP Care Management Team includes a social worker and a public health nurse, who coordinate, arrange and monitor services to meet medical care and social needs.

Read each program's description and eligibility criteria. To refer yourself or someone else to one of these programs, complete the online referral form or call (707) 565-5970.
