Half of American adults will become unable to speak for themselves during a future medical situation or crisis, yet only 23 percent have advanced health care directives defining our health care wishes for loved ones who will need to make decisions for us.
Get help to plan ahead and ensure that your medical care and end-of-life wishes are honored. Attend a free My Care, My Plan: Speak Up, Sonoma County workshop or presentation.
At the workshop, you will:
- think through the type of care you’d want in a health crisis,
- get tools to create a thoughtful advance care plan,
- learn how to complete clear, written, advance health care directives, and
- discuss the best way to talk with loved ones and health care providers about your wishes.
My Care, My Plan is an initiative of the Committee for Healthcare Improvement, the Sonoma County Human Services Department Adult and Aging Division and Sonoma County Health Action, My Care, My Plan mobilizes community partnerships and resources to achieve equity and improve health for all in Sonoma County.
