"Portraits" of Sonoma County Reflect on Impact

As a reflection on the Portrait's impact at the one-year anniversary from its release, community organizations participated in a social media project to literally "frame" how they're using the report in their work.

Overview of the Portrait

portrait-of-sonoma-county-200.jpgA Portrait of Sonoma County, released on May 20, 2014, is an in-depth look at how the residents of Sonoma County are faring in three fundamental areas: life expectancy, education, and income.  The report examines disparities within Sonoma County among neighborhoods and along the lines of race, ethnicity, and gender, and helps identify specific geographic places and populations in the County where we have the potential to positively affect long-term health outcomes. Health Action, a countywide collective impact initiative, uses A Portrait of Sonoma County and the report's Agenda for Action to inform Health Action's strategy in these areas:

The Portrait lays out concrete population and place-based recommendations (PDF) to respond to the report.
Health Action thumbnail 75 Local community health improvement groups are using the Portrait to create policy, system, and environmental change on a local level.

breathe-easy-75.jpgResponding to the Portrait's Agenda for Action, many communities are examining their tobacco ordinances to redouble anti-tobacco efforts.

Cradle to Career thumbnail 75 The community is leveraging the Portrait to advance the vision of Universal Preschool Access, including recently funding a Preschool Facilities Grant Program and being selected among five communities nationwide to receive technical assistance to develop innovative funding strategies.
